Meet the Vital Life Functional Medicine Team

Meneah Haworth, FNP-C, IFMCP
Provider and Owner
Meneah was born and raised on a farm in Maine. After high school she completed a semester course with Outward Bound in Colorado, hiking, white water rafting, canyoneering and rock climbing her way to a love for wellness. It was during that experience she realized that she wanted to help people achieve ultimate wellness in all aspects of life. She went on to earn her master's degree in Nursing in 2010. She started her journey with functional medicine in 2014, after her oldest brother passed away due to complications from colon cancer at 45 yrs of age. She wanted to understand why this had happened and embarked on a certification process with the Institute for Functional Medicine. Gaining full certification in 2018, Meneah began working with patients through a functional medicine lens. She now lives in Freeport, Maine with her husband, two daughters, dog and 12 chickens. To this day she absolutely loves that she is able to help her patients find wellness through modern medicine.

Skyler Laliberte, RN, Functional Medicine Nurse
Skyler Laliberte is a Functional Nurse with a passion for helping individuals take an active, empowered role in their health journey. With a strong foundation in critical care and clinical education, she brings both expertise and compassion to every patient interaction. Skyler received training in Functional Medicine through the Integrative Nurse Coaching Academy in partnership with the Institute for Functional Medicine.
She takes a whole-person, root-cause approach to care—focusing on uncovering and addressing the underlying imbalances that drive chronic health issues, rather than simply managing symptoms. Through the functional medicine model, Skyler helps restore health by using personalized nutrition and lifestyle interventions, functional lab testing, and targeted nutraceuticals. Skyler works closely with Meneah to address all the needs of the patient, including conventional medicine interventions as needed. She also educates patients on how their genetics, stress, environment, lifestyle habits, and emotional well-being impact overall health—and how to optimize each area for lasting wellness.
Skyler’s approach is collaborative, personalized, and grounded in the belief that true healing addresses the physical, mental, emotional, and spiritual dimensions of the individual. She is committed to guiding and educating her patients so they can make confident, informed choices and create a more vibrant, balanced life.

Kristin Caron
Office Manager
Kristin joins us with over 10 years of experience working in healthcare. She enjoys time outside by the ocean, hiking, or camping with her family.
Kristin will assist Meneah with messages and calls and be available throughout the week.
